Thank you very much. 00;01;45;26 - 00;01;47;09 Great to have you in here today. 00;01;47;09 - 00;01;47;23 So we're good. 00;01;47;23 - 00;01;51;13 The show today is about bioidentical hormone replacement. 00;01;51;14 - 00;01;53;17 We're going to talk about a lot of other things besides that. 00;01;53;23 - 00;01;57;09 We're going to talk about folks that issues with migraines. 00;01;57;28 - 00;01;59;24 We're going to talk about skin medica. 00;01;59;24 - 00;02;01;26 We're going to talk about bioidentical hormones. 00;02;02;01 - 00;02;03;18 We're going to talk about testosterone. 00;02;03;18 - 00;02;05;08 There's a lot we're going to do today. 00;02;05;08 - 00;02;06;25 Let me give a little background. 00;02;06;25 - 00;02;08;18 on Dr. Marc Bolan after 20 years. 00;02;08;18 - 00;02;12;26 Started a new practice Physician Aesthetic Specialist here in Kansas City area. 00;02;12;26 - 00;02;16;14 Physician Aesthetic Specialist is located in Lenexa, Kansas. 00;02;16;21 - 00;02;20;10 Physician Aesthetic Specialist serves the Greater Kansas City metro area, 00;02;20;16 - 00;02;23;08 Lenexa, Overland Park, Leeward, Olathe, etc.. 00;02;23;14 - 00;02;26;14 Dr. Margolin in surgical precision brings a new level 00;02;26;14 - 00;02;28;28 of professional performance to all esthetic treatments. 00;02;29;05 - 00;02;34;04 And Dr. Margolin decided to focus his skills on helping people feel their best 00;02;34;09 - 00;02;38;02 and live healthier lives through integrated healing of the body and mind. 00;02;38;02 - 00;02;40;12 And that's really kind of what this is all about today, folks. 00;02;40;12 - 00;02;43;22 So let's just start with kind of explain, Dr. 00;02;43;22 - 00;02;45;23 Bioidentical hormone replacement. 00;02;45;23 - 00;02;48;15 What is it and how does it work? 00;02;49;16 - 00;02;51;11 So bioidentical 00;02;51;11 - 00;02;54;10 hormone therapy is essentially 00;02;54;22 - 00;02;58;17 replacing the body's hormones that we lose as we get older. 00;02;59;10 - 00;03;03;05 It includes testosterone, estrogen, 00;03;03;05 - 00;03;05;21 progesterone, thyroid hormone. 00;03;07;01 - 00;03;12;27 This is really all about creating awareness because most people don't know 00;03;13;05 - 00;03;17;13 that as they get older, all of the signs and symptoms that we just 00;03;17;14 - 00;03;21;04 feel with aging are actually related to testosterone deficiency. 00;03;21;16 - 00;03;26;19 And women in particular don't understand that testosterone 00;03;26;19 - 00;03;30;17 is equally as important in them as it is in men. 00;03;30;28 - 00;03;36;15 And testosterone is the more predominant hormone in women, even over estrogen. 00;03;37;02 - 00;03;40;29 So it it's about creating the awareness that there's 00;03;40;29 - 00;03;46;00 something that we can do about it to make the aging process go a lot easier. 00;03;46;01 - 00;03;48;24 You know. That's that's the first time I've heard that. 00;03;48;24 - 00;03;51;15 And we've done a lot of shows around these kind of topics over the years, 00;03;51;25 - 00;03;55;01 but that clearly a lot of women out 00;03;55;01 - 00;03;57;29 there are listening to us right now are going what they know. 00;03;58;06 - 00;04;01;16 So when you so talk a little bit about that 00;04;01;16 - 00;04;04;22 because, you know, you see all the television commercials. 00;04;04;22 - 00;04;06;26 We'll get into that a little later on in the show 00;04;06;26 - 00;04;09;18 for all these testosterone products that are over the counter, 00;04;09;18 - 00;04;11;08 which I don't have a lot of faith in. 00;04;11;08 - 00;04;13;09 But again, I said we'll talk about that later. 00;04;13;09 - 00;04;17;10 But the idea that this is important for women as it is, men 00;04;17;29 - 00;04;19;05 kind of go into that a little bit. 00;04;19;05 - 00;04;20;15 Why is that the case? 00;04;20;15 - 00;04;21;17 And again, 00;04;21;17 - 00;04;25;29 you know, this goes back to as you age, as you become chronologically challenged. 00;04;25;29 - 00;04;28;14 I'm 73 years old this month. Okay. 00;04;29;04 - 00;04;34;01 You know, your hormones, these levels start to drop and then what happens then? 00;04;34;05 - 00;04;36;09 So how does that relate directly to women? 00;04;36;09 - 00;04;36;19 Okay. 00;04;36;19 - 00;04;39;28 So first, just to explain about bioidentical, 00;04;39;28 - 00;04;42;20 what that means is that there are synthetic hormones 00;04;43;21 - 00;04;45;16 and there are bioidentical. Biodata Identical 00;04;45;16 - 00;04;50;13 means that we are replacing the body with the same hormones 00;04;50;20 - 00;04;51;28 that our body produces. 00;04;51;28 - 00;04;56;23 So we recognize them as self as opposed to synthetic hormones, 00;04;56;23 - 00;05;00;28 which the body recognizes as foreign and doesn't respond to as well. 00;05;00;28 - 00;05;04;05 And so there are there are a lot of myths and concerns 00;05;04;05 - 00;05;07;23 about hormone therapy and cancer, for example. 00;05;08;21 - 00;05;11;23 And it was all based on synthetic hormones and mainly around 00;05;11;28 - 00;05;16;00 Progestins and the drug prime pro in women. 00;05;16;03 - 00;05;19;13 It was through the Women's Health Care Initiative in the early 2000s 00;05;19;13 - 00;05;22;01 that scared a lot of people off from doing hormones. 00;05;22;20 - 00;05;26;28 So we need to break those myths and so the first thing to start with 00;05;26;28 - 00;05;29;28 is understanding the signs and symptoms that we can fix 00;05;30;25 - 00;05;34;04 equally in men and women with testosterone. So 00;05;35;15 - 00;05;36;02 loss of 00;05;36;02 - 00;05;40;14 libido as we get older or inability to perform sexually. 00;05;40;14 - 00;05;45;01 And these are really important things that cause stress in marriages because 00;05;45;01 - 00;05;48;14 you might have one partner that has a stronger sexual drive than the other 00;05;48;23 - 00;05;51;13 and it can truly destroy a marriage. 00;05;52;08 - 00;05;53;06 Sleep disturbance. 00;05;53;06 - 00;05;57;29 So as we get older, waking up to have coffee at 4 a.m. 00;05;57;29 - 00;05;59;27 rather than 6 a.m.. 00;05;59;27 - 00;06;01;06 That sound familiar? 00;06;01;06 - 00;06;02;23 Yeah, it. Does, unfortunately. 00;06;02;23 - 00;06;03;06 I try to. 00;06;03;06 - 00;06;05;00 I try to make it around, get to five. 00;06;05;00 - 00;06;07;11 But, yeah, I know exactly what you're talking about. Right. 00;06;07;17 - 00;06;11;21 Or as you get older, waking up every hour of the night 00;06;11;21 - 00;06;14;25 to go pee, which might not be a prostate problem. 00;06;14;25 - 00;06;16;09 It could be a testosterone problem. 00;06;16;09 - 00;06;19;13 Now, that's interesting because that's always identified as a prostate problem. 00;06;19;13 - 00;06;21;02 And you're saying. Not necessarily. 00;06;21;02 - 00;06;22;20 Not necessarily so. 00;06;22;20 - 00;06;25;02 And part of my workup includes 00;06;25;21 - 00;06;28;04 examining your your PSA level. 00;06;28;12 - 00;06;34;16 So which is the prostate specific antigen, which can be a telltale for either 00;06;34;29 - 00;06;38;23 benign prostatic hypertrophy or even prostate cancer. 00;06;39;01 - 00;06;41;19 So that's kind of part of my comprehensive screening. 00;06;42;01 - 00;06;45;02 So some of the other some of the other symptoms. 00;06;45;29 - 00;06;50;05 So we've covered sex, we've covered sleep disturbance. 00;06;50;26 - 00;06;53;06 As we get older, it gets more difficult 00;06;53;06 - 00;06;56;18 to lose weight and specifically belly fat. 00;06;56;18 - 00;07;01;28 So that so testosterone is responsible for people's inability to lose the belly 00;07;01;28 - 00;07;05;25 fat like, gosh, doc, like I exercise like crazy, I diet like crazy, 00;07;05;25 - 00;07;09;22 but I can't lose the gut specifically and that's testosterone. 00;07;09;22 - 00;07;11;23 Or you start lifting weights 00;07;11;28 - 00;07;15;15 and why can't I get biceps again like I did when I was 25? 00;07;15;24 - 00;07;18;11 It's testosterone. Also mood disturbance, 00;07;19;10 - 00;07;21;24 irritability, moodiness. 00;07;21;24 - 00;07;24;12 A lot of people are on an antidepressant, 00;07;24;12 - 00;07;27;18 specifically SSRI I's, and 00;07;28;23 - 00;07;32;23 this is not necessarily a cure for everyone's depression 00;07;32;23 - 00;07;37;13 because they might have genetically major anxiety, depression disorder. 00;07;37;13 - 00;07;38;13 But as we get older 00;07;38;13 - 00;07;41;22 and have difficulties with our mood and kind of keeping that upbeat 00;07;42;22 - 00;07;46;08 outlook on life, it's related to testosterone. 00;07;47;09 - 00;07;50;12 The that feeling after lunch 00;07;50;12 - 00;07;54;26 where you just can't fight that feeling that I cannot go on if I don't take a nap. 00;07;55;13 - 00;07;58;20 Yep. That's that is a that's a real big one. 00;07;59;03 - 00;08;02;10 So these are all examples of things 00;08;02;10 - 00;08;05;22 that as we get older, you just say geez, 00;08;05;22 - 00;08;10;12 This just I guess it's just because I'm getting older and people 00;08;10;12 - 00;08;13;00 just kind of accept that as their way of life. 00;08;14;13 - 00;08;18;28 When we look at women, women blame everything on the on 00;08;18;28 - 00;08;22;10 estrogen for symptoms around menopause, 00;08;23;04 - 00;08;25;11 not knowing that 00;08;25;11 - 00;08;28;16 testosterone is more responsible than estrogen 00;08;29;04 - 00;08;34;16 for creating the perimenopausal symptoms like hot flashes and night sweats. 00;08;36;07 - 00;08;39;02 You know, some of the headaches 00;08;39;02 - 00;08;41;25 it's driven at least as much, 00;08;41;25 - 00;08;44;27 if not more by testosterone than is by estrogen. 00;08;45;09 - 00;08;47;06 Yeah, this is fascinating. 00;08;47;06 - 00;08;50;29 I mean, just to listen to you go through this list from, you know, sleep 00;08;50;29 - 00;08;54;10 disturbance, weight issues, mood, irritability, 00;08;55;02 - 00;08;57;17 you know, the need for the nap after lunch type of thing. 00;08;57;29 - 00;09;00;02 It is quite remarkable. 00;09;00;02 - 00;09;03;14 All the pieces that kind of come together under this level of testosterone. 00;09;03;19 - 00;09;05;12 Obviously, we've got a lot to talk about. 00;09;05;12 - 00;09;07;01 We're going to be right back after the break. 00;09;07;01 - 00;09;10;17 You're listening to America's Healthcare Advocate 00;09;10;17 - 00;09;14;03 broadcasting here on the HIA radio network. Coast to coast across the USA. 00;09;14;04 - 00;09;15;08 We'll be right back with more. 00;09;15;08 - 00;09;20;04 The doctor is in the house with you. 00;09;20;04 - 00;09;21;13 In studio with me, Dr. 00;09;21;13 - 00;09;23;19 Dan Margolin, our producer today. Mr. 00;09;23;19 - 00;09;24;09 Darren Willhite. 00;09;24;09 - 00;09;26;29 I'm your host, Cary Hall, the website for 00;09;26;29 - 00;09;29;28 Dr. Margolin. He's got a great website. There's a lot of information up there. 00;09;29;28 - 00;09;33;22 If you want to go up there, it's physicianaestheticspecialists.com 00;09;33;22 - 00;09;36;22 physicianaestheticspecialists.com 00;09;36;28 - 00;09;43;14 The phone numbers 913-706-0088, 913-706-0088. 00;09;43;21 - 00;09;46;06 So off the air, you were talking about women in menopause. 00;09;46;06 - 00;09;48;29 Let's let's go to that right now and just talk about 00;09;49;09 - 00;09;53;03 how you perceive that and what your treatment regimen for. 00;09;53;03 - 00;09;54;07 That is when somebody comes 00;09;54;07 - 00;09;57;08 in, they're in the middle of menopause that they're not whatever the case may be. 00;09;57;08 - 00;09;57;21 Doctor. 00;09;57;21 - 00;10;01;22 So it's interesting the asset, because yesterday 00;10;01;23 - 00;10;04;22 I had two women that called and 00;10;04;28 - 00;10;09;09 I did a screening consultation with them over the phone and make it easy for them. 00;10;09;09 - 00;10;14;11 And both women were in menopause and did not even realize it. 00;10;14;11 - 00;10;18;25 And unfortunately, since COVID, I hear a frequent story that 00;10;19;02 - 00;10;24;03 my doctor retired or the practice closed and they haven't found a new doctor. 00;10;24;03 - 00;10;27;07 So they're it's kind of an excuse that they're not taking care of themselves. 00;10;27;18 - 00;10;30;18 The telltale is when I do their screening labs 00;10;30;27 - 00;10;34;14 before their procedure, I look at their FSH level. 00;10;34;19 - 00;10;38;19 And if their fsh level is in a certain range, it's diagnostic 00;10;38;29 - 00;10;40;18 for being in menopause. 00;10;40;18 - 00;10;44;04 And it's blows my mind that there's so many people 00;10;44;04 - 00;10;46;25 that have no idea that they are actually in menopause. 00;10;47;06 - 00;10;49;26 You know, I'll ask them, well, when was your last menstrual period? 00;10;50;06 - 00;10;51;05 And I've had a lot of 00;10;52;09 - 00;10;52;18 I have 00;10;52;18 - 00;10;56;19 no idea or it's been at least six months, but they haven't done anything about it. 00;10;56;19 - 00;10;59;12 And that's an integral part of my treatment. 00;10;59;21 - 00;11;02;26 And we have to look at are they still having menstrual cycles? 00;11;03;04 - 00;11;05;06 Have they had a hysterectomy? 00;11;05;06 - 00;11;08;27 So do they have their uterus because women's implants, 00;11;08;27 - 00;11;13;25 if they are post-menopausal, they're getting testosterone 00;11;13;25 - 00;11;17;18 and estrogen pellets as part of their implant. 00;11;17;23 - 00;11;20;21 So if they still have a uterus, then 00;11;21;10 - 00;11;25;13 they need micronised progesterone in in the form of a pill. 00;11;25;13 - 00;11;28;09 But I use a bioidentical progesterone. 00;11;28;09 - 00;11;30;12 That's what the micronized progesterone is so 00;11;30;12 - 00;11;33;13 that it's all completely safe and healthy. 00;11;34;09 - 00;11;38;21 So we really have to tailor our treatment to, you know, 00;11;38;28 - 00;11;42;27 women's female organs and where they are and the state of menstrual cycles. 00;11;42;27 - 00;11;44;28 So it's very specific. 00;11;44;28 - 00;11;46;25 So again, and everybody's different. 00;11;46;25 - 00;11;50;09 So when they come in to your practice, we'll go to men here in a little while. 00;11;50;09 - 00;11;51;23 But when they come into your practice, 00;11;51;23 - 00;11;55;16 you do a complete blood panel on them and that's how you start to determine 00;11;55;16 - 00;11;57;27 what is not there and what needs to be there. 00;11;57;28 - 00;12;01;06 Well, so essentially this is interesting. 00;12;01;06 - 00;12;08;16 So men starting at age 25, start losing about 1% to 3% of their testosterone 00;12;08;16 - 00;12;11;09 per year, somewhere between the age of 25 and 30. 00;12;11;25 - 00;12;15;12 It's roughly the same for women, but women by the age of 40 00;12;15;12 - 00;12;18;19 have lost at least 50% of their testosterone. 00;12;18;20 - 00;12;20;02 Holy cow. Yes. 00;12;20;02 - 00;12;21;10 By the age of 40. 00;12;21;10 - 00;12;23;12 By the age of 40. So actually, 00;12;24;11 - 00;12;24;27 I never was 00;12;24;27 - 00;12;28;20 this great at math, but it seems to be that women are losing it 00;12;28;20 - 00;12;32;25 at a faster rate than men are by the time you get to 40, 45, 50. 00;12;32;25 - 00;12;35;06 Am I right or wrong? That could be okay. 00;12;36;04 - 00;12;39;26 Because the emphasis is always men losing it, right. 00;12;39;26 - 00;12;43;13 And all the advertising and everything you see is focused on men. 00;12;43;19 - 00;12;47;19 But in reality, women either are on the same track or maybe even a little ahead. 00;12;47;23 - 00;12;51;07 By the time you get to 40, 45 years of age, whatever the case may be. 00;12;51;07 - 00;12;52;22 You know how the there's 00;12;52;22 - 00;12;55;12 the old saying that when you turn 40, it's all downhill from there. 00;12;55;13 - 00;12;56;10 Right? 00;12;56;10 - 00;12;57;14 It's your testosterone. 00;12;57;14 - 00;12;59;07 It's all downhill from there. 00;12;59;07 - 00;13;02;03 That's interesting. I remember that. Yeah. 00;13;02;03 - 00;13;08;24 So the basically the way that we do it is a screening intake form 00;13;08;24 - 00;13;13;20 that is basically a ranking checklist to ask a patient 00;13;14;06 - 00;13;16;17 a list of all the symptoms and where they ranked it. 00;13;17;00 - 00;13;18;18 And essentially 00;13;19;23 - 00;13;22;24 every everyone, once they're 00;13;22;24 - 00;13;26;29 into their mid-thirties or forties, is a candidate to some degree. 00;13;27;05 - 00;13;31;06 So really all you need to be a candidate for bioidentical hormone 00;13;31;06 - 00;13;34;11 therapy is to be able to just start checking the boxes. 00;13;34;20 - 00;13;36;08 So when they come in, you basically, I'm 00;13;36;08 - 00;13;39;03 guessing the box goes, are you having sleep problems 00;13;40;01 - 00;13;43;11 or do you have a weight issue with belly fat and you're working out 00;13;43;11 - 00;13;44;20 trying like hell to get rid of it? 00;13;44;20 - 00;13;46;20 It's just not happening. 00;13;46;20 - 00;13;51;26 You know, mood issues, mood swings, that that afternoon nap thing after lunch. 00;13;51;26 - 00;13;55;21 I mean, is that all if you're you know, those are just some of the things. 00;13;55;21 - 00;13;59;16 I'm sure there's a lot more to talk about, but you're basically getting a survey 00;13;59;16 - 00;14;01;14 from them about where they're at healthwise. 00;14;01;14 - 00;14;01;28 That's right. 00;14;01;28 - 00;14;02;27 We asked them to rank it. 00;14;02;27 - 00;14;05;07 Mild, moderate, severe, very severe. 00;14;05;07 - 00;14;09;02 And then so that's step one of the awareness. 00;14;09;02 - 00;14;12;29 And then it's a matter of, you know, some some people are already 00;14;12;29 - 00;14;16;10 well educated and they're ready to enter the program. 00;14;16;10 - 00;14;18;29 And other people, it's just a matter of sitting down and having 00;14;19;15 - 00;14;21;16 a regular conversation with them. 00;14;21;16 - 00;14;24;21 So then step two is that we do 00;14;24;21 - 00;14;27;24 the blood work and I do the blood work in my office. 00;14;27;24 - 00;14;30;17 So, yeah. You're sitting about to a lab. You do it right there. 00;14;30;17 - 00;14;33;17 Yeah, everything I do, I want to make it easy for the patient. 00;14;33;19 - 00;14;39;13 Okay, so online booking, if someone reaches out to me directly, I'll say, hey, 00;14;40;04 - 00;14;43;20 I can I can book your appointment for you, because technology makes it that easy. 00;14;43;20 - 00;14;43;28 Yeah. 00;14;43;28 - 00;14;47;03 Is, you know, I'm all about the customer service making it easy. 00;14;47;03 - 00;14;51;07 So I don't want to send people out to have to get to a lab, go someplace else. 00;14;51;07 - 00;14;54;04 They go to LabCorp. Very simple book an appointment. 00;14;54;04 - 00;14;56;06 I do the labs in the office. 00;14;56;13 - 00;15;00;25 All the labs are really doing is guiding me as far as the dosage. 00;15;00;25 - 00;15;06;09 So I have a program, I'm a certified Biote provider. 00;15;06;09 - 00;15;11;09 Biote is the the company that I use to do bioidentical hormone therapy. 00;15;11;19 - 00;15;13;13 So I have a program. 00;15;13;13 - 00;15;15;25 So I get all of the lab values 00;15;16;23 - 00;15;18;24 and I plug those lab values in 00;15;19;01 - 00;15;24;02 and it makes recommendations, guideline recommendations for me. 00;15;24;02 - 00;15;26;10 For what dosages of specific. 00;15;26;10 - 00;15;29;26 To that. specific to that person. Wow. 00;15;30;01 - 00;15;31;20 And so I plug it in. 00;15;31;20 - 00;15;33;19 It tells me how much testosterone they need. 00;15;33;19 - 00;15;37;14 If it's a woman, you know, I know by entering it in the program, 00;15;38;00 - 00;15;39;23 are they still having menstrual cycles? 00;15;39;23 - 00;15;41;10 Have they had a hysterectomy? 00;15;41;10 - 00;15;43;15 Have they had cancer? 00;15;43;27 - 00;15;46;29 So that I so I can't miss the mark. 00;15;47;18 - 00;15;47;26 Right. 00;15;47;26 - 00;15;53;10 So then that so the labs don't tell me if you need to be in the program. 00;15;53;18 - 00;15;55;18 You're telling me if you need to be in the program. 00;15;55;27 - 00;15;59;08 Right, the labs just tell me how I'm going to dose you 00;15;59;18 - 00;16;04;17 and what supplements I'm going to give you after your pellet implants. 00;16;04;17 - 00;16;05;17 And then. By the way. 00;16;05;17 - 00;16;08;04 All right, so you're thinking, oh, my God, how big is this thing? 00;16;08;04 - 00;16;09;10 They're going to what? 00;16;09;10 - 00;16;11;06 I've seen the pellet online. 00;16;12;05 - 00;16;12;27 How big is that? 00;16;12;27 - 00;16;15;16 I mean, okay. So it it depends on the dose. Okay. 00;16;15;17 - 00;16;19;18 But and and so let's say estrogen, for example, is 00;16;19;27 - 00;16;25;03 the estrogen pellet is smaller than than the smallest pill you've ever taken. 00;16;25;04 - 00;16;27;23 Yeah, it looks. Like about a Rice Krispie to me is what I. Yeah, yeah. 00;16;27;24 - 00;16;30;00 Or some of them are even smaller. Yeah. 00;16;30;00 - 00;16;32;24 The testosterone pellets, the largest size 00;16;32;24 - 00;16;36;19 pellet is essentially kind of like a rabbit food pellet. 00;16;36;20 - 00;16;37;16 Okay. 00;16;37;28 - 00;16;40;29 They are all implanted in the subcutaneous 00;16;40;29 - 00;16;46;13 fat in the in kind of the the hip or buttocks region. 00;16;46;14 - 00;16;48;21 Okay. It's done through a tiny incision. 00;16;49;03 - 00;16;53;15 And I slide them in through a trocar, which is essentially like a hollow needle 00;16;53;17 - 00;16;56;16 right now, inject a little numbing medication, small incision, 00;16;57;15 - 00;16;59;02 slide the implants in, and 00;16;59;02 - 00;17;03;10 it's literally just a steady strip Band-Aid over it. 00;17;03;10 - 00;17;05;02 And then this release is over it. 00;17;05;02 - 00;17;07;11 It releases. Over a. Period of time. Right. Okay. 00;17;07;11 - 00;17;09;29 So the idea is that 00;17;10;13 - 00;17;13;08 so I'm going to bring you back four weeks after your implant. 00;17;13;08 - 00;17;15;26 Right. We'll pick it right up. And we come back after the break. 00;17;15;26 - 00;17;19;22 You're listening to America's Healthcare Advocate broadcasting here on the HIA 00;17;19;23 - 00;17;22;16 radio network, coast to coast across the U.S. 00;17;22;16 - 00;17;24;02 We'll be right back with the doctors. 00;17;24;02 - 00;17;27;29 Stay tuned. 00;17;36;15 - 00;17;38;20 Welcome back. Okay. 00;17;51;19 - 00;17;54;08 I mean, doctors giving a lot of information out here. 00;17;54;08 - 00;17;57;16 They like drinking from a firehose if you're listening to all this. 00;17;57;16 - 00;18;00;28 So if you want to tell you maybe your wife has tried this before 00;18;01;05 - 00;18;01;27 and we're going to talk a little 00;18;01;27 - 00;18;05;16 about some of those other treatments that hasn't worked or maybe, you know, 00;18;05;23 - 00;18;08;21 you want to tell somebody about it or you won't tell your husband about it. 00;18;10;03 - 00;18;12;27 Again, the podcast platform, there's your opportunity. 00;18;12;27 - 00;18;13;14 Okay. 00;18;13;14 - 00;18;17;08 Also YouTube at HealthRadio.us 00;18;17;08 - 00;18;21;12 the new website HealthRadio.us you add Dr. Margolin's 00;18;21;13 - 00;18;25;23 office practice is physicianaestheticspecialists.com plural 00;18;25;27 - 00;18;29;00 physicianaestheticspecialists.com 00;18;29;07 - 00;18;32;19 Phone number 9137060088. 00;18;32;26 - 00;18;36;22 If you want to go on the website and take a look where they do, it's pretty cool. 00;18;36;29 - 00;18;38;16 All right. And if you want to go in 00;18;38;16 - 00;18;40;22 and have a consultation, you heard him say he can do it. 00;18;40;22 - 00;18;43;05 You know, they can do a Zoom conference if you want to do that. 00;18;43;05 - 00;18;46;05 So it's a great opportunity just to see if you're dealing 00;18;46;05 - 00;18;49;17 with any of this stuff and you are chronologically challenged. 00;18;49;17 - 00;18;51;14 Okay. You're a seasoned citizen. 00;18;51;14 - 00;18;53;16 This is an opportunity to do something about it. 00;18;53;16 - 00;18;54;28 And it's not drugs. 00;18;54;28 - 00;18;57;01 And that's that's a big difference in this country. 00;18;57;01 - 00;18;59;08 You know, we love our pharmaceuticals. 00;18;59;14 - 00;19;01;24 Well, this is not drugs. It's a different deal. 00;19;02;03 - 00;19;03;18 All right, Doctor, let's go back to this. 00;19;03;18 - 00;19;05;20 So for weeks now, what happens? 00;19;05;29 - 00;19;10;25 The patient comes back in and they will have their blood drawn again 00;19;10;25 - 00;19;14;17 so we can see where their levels are at and they'll fill out the same survey form 00;19;14;19 - 00;19;20;06 that they did, their intake form that they did before entering the program. 00;19;20;06 - 00;19;23;08 If they don't have a significant improvement in their score, 00;19;23;09 - 00;19;27;14 then I offer to do a boost pellet on them 00;19;27;14 - 00;19;32;09 to get their testosterone up or get their a woman's estrogen up a little bit. 00;19;32;09 - 00;19;35;07 If if they need if we need to do that, 00;19;36;15 - 00;19;38;02 typically women will come 00;19;38;02 - 00;19;42;04 back for their second round of pellets at four months and men are at five months. 00;19;42;04 - 00;19;46;29 The idea is that we are going to achieve a steady state to where we have 00;19;46;29 - 00;19;50;25 their testosterone, estrogen, levels up 00;19;50;25 - 00;19;52;29 to where we want them to be 00;19;54;02 - 00;19;55;19 for forever. 00;19;55;19 - 00;20;00;10 And we know that that's about the period of time before it needs to be replaced. 00;20;00;10 - 00;20;04;25 However, if they feel like it's starting to taper off a little bit, 00;20;04;25 - 00;20;06;00 just come on right back in. 00;20;06;00 - 00;20;10;20 And we know that you might be someone that needs another pellet insertion done 00;20;10;22 - 00;20;12;27 a little sooner because everyone is different. 00;20;13;21 - 00;20;17;13 This is very different than synthetic hormone replacement. 00;20;17;13 - 00;20;19;04 Yeah, I want to get into that 00;20;19;04 - 00;20;23;19 because there's a lot of bad press out there around Synthetic. 00;20;23;27 - 00;20;24;09 Okay. 00;20;24;09 - 00;20;27;23 And so talk about that a little bit because this is this is all natural. 00;20;27;23 - 00;20;30;13 It is not synthetic. And that's important. 00;20;30;13 - 00;20;32;23 People are tend to talk about that a little bit. 00;20;32;23 - 00;20;36;19 So it goes back to the early 2000s with the Women's Health Care Initiative, 00;20;37;07 - 00;20;41;25 where there was a direct link made between hormone 00;20;41;25 - 00;20;45;22 replacement therapy for women and breast cancer. 00;20;45;25 - 00;20;46;19 Okay. 00;20;46;24 - 00;20;49;28 That is that stuck around 00;20;50;09 - 00;20;55;21 for over 20 years before those studies were basically debunked 00;20;56;09 - 00;21;01;13 and that but a scare sticks around a lot longer, 00;21;01;23 - 00;21;05;19 you know, then of course, then being able to dispel the myth they're all 00;21;05;22 - 00;21;09;15 and now in fact, there are two recent large studies that have come out 00;21;09;15 - 00;21;14;02 that shows that not only does bioidentical hormone therapy not cause cancer, 00;21;14;12 - 00;21;19;09 but there are two major studies that have shown a 35% reduction 00;21;19;09 - 00;21;22;28 in breast cancer associated with bioidentical hormone therapy. 00;21;23;18 - 00;21;26;23 If we're really doing comprehensive hormone therapy, 00;21;26;23 - 00;21;31;20 there's there's also up to a 2 to 3 fold decreased risk of Alzheimer's. 00;21;31;20 - 00;21;35;18 Now, I saw that on the website, and I was going to go right and ask you about that. 00;21;35;24 - 00;21;37;08 So do that one more time. 00;21;37;08 - 00;21;38;12 I want people to hear that. 00;21;38;12 - 00;21;43;20 2 to 3 fold, decreased risk of Alzheimer's. Wow. 00;21;44;07 - 00;21;49;24 So I mean that that in and of itself is pretty remarkable when you think about it. 00;21;49;24 - 00;21;50;09 Okay. 00;21;51;21 - 00;21;54;16 And so what do you say to people? 00;21;54;23 - 00;21;57;10 You know, we I did this, you know, what is coming to the brain. 00;21;57;10 - 00;22;01;18 But what do you say to people say, hey, you know, I tried this. 00;22;01;18 - 00;22;04;10 I was taking oral hormones or I was doing it. 00;22;04;10 - 00;22;08;04 It didn't work, yada, yada, yada, yada, yada. 00;22;08;14 - 00;22;11;04 How do you respond to that in terms of, well, 00;22;11;04 - 00;22;13;26 this is a different deal and talk about how you. 00;22;14;13 - 00;22;17;17 I have had that come up several times just in the past week. 00;22;19;03 - 00;22;20;02 How has. That. 00;22;20;02 - 00;22;24;25 Yeah, so so anything that is not 00;22;25;10 - 00;22;28;02 bioidentical hormone therapy is is synthetic 00;22;28;12 - 00;22;31;08 and there are different forms of giving it 00;22;31;10 - 00;22;37;03 and it's all inconsistent and it's dependent upon absorption in the body. 00;22;37;03 - 00;22;41;28 So went for women they'll they'll do estrogen creams that are they're 00;22;41;28 - 00;22;44;28 vaginal creams they are skin patches 00;22;44;28 - 00;22;48;25 as there are pills, there are lozenges. 00;22;48;25 - 00;22;51;04 And by the way, it's the pills, like we mentioned earlier, 00;22;51;04 - 00;22;55;11 the synthetic progestins that were the ones that linked 00;22;56;10 - 00;22;58;19 hormone therapy to cancer in women. 00;22;58;29 - 00;23;03;00 So we do not use anything, 00;23;03;00 - 00;23;06;14 synthetic everything that I give and supplement 00;23;07;03 - 00;23;12;04 pill prescription and the pellet implants is all bioidentical. 00;23;12;04 - 00;23;15;11 So it only has positive health effects. 00;23;16;01 - 00;23;19;06 So if you're putting a cream on 00;23;19;11 - 00;23;23;24 for a woman on the vagina or even a patch on the skin or there's 00;23;23;24 - 00;23;28;12 been testosterone creams that you rub on, it is all absorption dependent. 00;23;28;22 - 00;23;32;10 And that's compound dependent based on like the manufacturer, 00;23;32;19 - 00;23;36;25 the pharmacy that has to compound a testosterone injection, 00;23;37;07 - 00;23;40;06 which is the common thing that men are after. 00;23;40;19 - 00;23;45;15 And the the levels will vary from day to day. 00;23;45;27 - 00;23;48;03 So if a woman 00;23;48;03 - 00;23;51;09 like I've had in the past, you know, three of them in the past week, 00;23;51;09 - 00;23;54;00 if they tell me hormone therapy didn't work for me, 00;23;54;13 - 00;23;58;08 it's it is it's not that the hormone therapy didn't work for them. 00;23;58;08 - 00;24;01;26 It's that it either wasn't being managed correctly 00;24;02;09 - 00;24;05;20 or they weren't getting the right form. 00;24;05;20 - 00;24;10;01 It wasn't individualized for them based on labs and all the things that you do, 00;24;10;06 - 00;24;11;16 which are very, very different 00;24;11;16 - 00;24;15;04 than we're going to throw much of it up against the wall and see what sticks. 00;24;15;04 - 00;24;17;21 Right. This is radio or I'd say something different. 00;24;19;29 - 00;24;22;00 So let's switch gears a minute. 00;24;22;00 - 00;24;26;07 You see all these commercials on television with for men, okay. 00;24;26;13 - 00;24;31;01 With testosterone and place, order Iron Man today or what? 00;24;31;01 - 00;24;32;22 I'm making this stuff up, people. 00;24;32;22 - 00;24;34;19 I mean, I don't remember the name of all those crap. 00;24;34;19 - 00;24;35;29 I see it on television today. 00;24;35;29 - 00;24;39;16 You see these athletes that are all buffed up and they're talking about, boy, 00;24;39;16 - 00;24;44;14 this did this for me, or I played golf now or I can do this. 00;24;44;14 - 00;24;47;00 That rings very hollow to me. Okay. 00;24;47;20 - 00;24;51;03 First of all, how do you know when you're taking something? 00;24;51;03 - 00;24;52;28 You're ordering this stuff, you know, 00;24;52;28 - 00;24;56;00 and today in the day, only at whatever the price is right. 00;24;56;00 - 00;24;58;05 And we'll ship you a year's worth or whatever it is 00;24;58;26 - 00;25;01;05 that you're not being tested for anything. 00;25;01;09 - 00;25;03;05 You're putting your stuff in your body. 00;25;03;05 - 00;25;06;10 You have no idea if that's the right amount, the wrong 00;25;06;10 - 00;25;10;19 amount, too damn much it's going to cause another problem I mean and is that 00;25;10;25 - 00;25;15;14 and is it even real testosterone that you're putting in your body, Doctor? 00;25;15;23 - 00;25;16;25 No, it's not. 00;25;16;25 - 00;25;18;09 There's your answer, people. 00;25;18;09 - 00;25;23;01 Any anything, anything that you can buy, retail 00;25;23;01 - 00;25;27;22 that you can buy yourself has no FDA regulation, 00;25;29;10 - 00;25;31;20 which means that 00;25;31;20 - 00;25;36;10 they can tell you that it is 1 million 00;25;36;10 - 00;25;41;02 milligrams of testosterone or anything. 00;25;41;02 - 00;25;43;18 There's no one to regulate to say there's anything. 00;25;44;09 - 00;25;46;16 You know, it could be chewing gum. And 00;25;48;15 - 00;25;50;03 but some of it may well 00;25;50;03 - 00;25;53;22 be, but there's no FDA regulations, so they can call it whatever they want 00;25;53;22 - 00;25;57;08 and they can sell it and bottle it however they want to. 00;25;58;00 - 00;26;01;05 And it does not mean that that's what you're getting. 00;26;01;15 - 00;26;05;01 And, you know, in the past, I've even been fooled by some of that 00;26;05;01 - 00;26;10;04 because you're so just desperate to take a vitamin for this or that or, you know, 00;26;10;15 - 00;26;14;26 we've got a culture where we just we we throw a drug at everything. 00;26;15;17 - 00;26;15;27 Yeah. 00;26;15;27 - 00;26;19;09 Rather than just actually what we're what I am doing 00;26;19;09 - 00;26;26;09 is treating the true cause of the issues, which is replace the hormones 00;26;26;22 - 00;26;31;07 as I think a really good point is that people will ask, well, 00;26;31;07 - 00;26;33;22 how long do I have to stay on this therapy? 00;26;34;07 - 00;26;34;27 You know. 00;26;35;09 - 00;26;38;27 I got a better why wouldn't I want to stay on this therapy? 00;26;38;27 - 00;26;44;27 If I go through this list and I look at these issues weight, sleep, mood, you know, 00;26;46;00 - 00;26;49;04 sexual performance going down the list, who go? 00;26;49;04 - 00;26;51;28 My question would be, why the hell wouldn't I want to stay on this? 00;26;51;28 - 00;26;55;22 And and that is that is my answer to people's question. 00;26;55;22 - 00;26;56;27 But I think the first thing 00;26;56;27 - 00;27;00;27 that they really just don't understand is that I have to explain that 00;27;01;08 - 00;27;05;13 as we get older, we lose the ability to make these hormones 00;27;05;21 - 00;27;10;06 and there is no medication that can make our body 00;27;10;10 - 00;27;16;08 start producing the testosterone again or start producing the estrogen again. 00;27;16;22 - 00;27;19;11 So it's a matter of replacing it. 00;27;19;11 - 00;27;24;08 And as we get older, it's just a matter of how do we choose to get older? 00;27;24;08 - 00;27;29;03 And once people get started in that program, their not going to want... 00;27;29;21 - 00;27;31;24 well, it's quality of. Life, it is quality of life. 00;27;31;24 - 00;27;34;29 You know, I go back to your little mission statement here when you talk about, 00;27;35;06 - 00;27;38;25 you know, focusing your skills on helping people feel their best 00;27;39;11 - 00;27;43;04 and live healthier lives through integrated healing body and mind. 00;27;43;04 - 00;27;45;14 Well, that kind of sums it up. 00;30;04;27 - 00;30;05;20 That's right. 00;30;05;20 - 00;30;08;26 I think that men don't take care of themselves as well as women. 00;30;08;26 - 00;30;10;04 And we know that. 00;30;10;04 - 00;30;14;12 But men also don't like to admit when there is a problem. 00;30;14;12 - 00;30;17;04 We don't share our feelings the way women do 00;30;18;08 - 00;30;20;15 because we don't have as much estrogen as they do. 00;30;20;15 - 00;30;23;03 And that's that's that for that's it. 00;30;23;03 - 00;30;23;13 That's it. 00;30;23;13 - 00;30;27;06 It's however, it's it's really funny because if you think about it as you're 00;30;27;06 - 00;30;30;21 losing testosterone, you are kind of losing some of your manlyhood 00;30;31;02 - 00;30;34;02 when it comes to things like sexual performance or, 00;30;34;29 - 00;30;37;05 you know, belly fat loss of muscle mass. 00;30;37;12 - 00;30;37;29 Right. 00;30;37;29 - 00;30;41;21 So men don't want to admit that they're losing their manlyhood, 00;30;42;03 - 00;30;45;08 which is basically admitting that they need testosterone. 00;30;45;10 - 00;30;46;28 They just don't know that. 00;30;46;28 - 00;30;47;07 Yeah. 00;30;47;07 - 00;30;50;07 And, you know, they do that. 00;30;50;07 - 00;30;54;12 And then the you know, and then and then they start to have other issues, 00;30;54;12 - 00;30;56;28 you know, maybe maybe it's problems in the marriage. 00;30;56;28 - 00;30;57;15 Okay. 00;30;57;15 - 00;31;00;19 Maybe you're biting your wife's head off every time you turn around or you're 00;31;00;19 - 00;31;03;24 tired all the time and it's difficult to function. 00;31;03;28 - 00;31;06;01 And all of that ties right back to this. 00;31;06;08 - 00;31;10;29 And you and the other thing you do is you go see your primary care doctor. 00;31;11;08 - 00;31;13;11 Hey, we'll give you a sleeping pill. 00;31;13;11 - 00;31;17;25 Give me, give you so that Trazodone see how that works for you or Will, you're. 00;31;17;26 - 00;31;19;27 Man, you've got a mood problem. How about some Prozac? 00;31;20;03 - 00;31;22;20 You're putting a Band-Aid on it. You're not solving the problem. 00;31;22;20 - 00;31;26;08 Your your testosterone level is going to continue to decrease. 00;31;26;13 - 00;31;29;00 And the drugs aren't doing the job. Am I right or wrong? 00;31;29;01 - 00;31;29;18 That's right. 00;31;29;18 - 00;31;33;14 And it is I am very hopeful with all the patients, 00;31;33;14 - 00;31;37;11 especially ones that are on SSRI's for basic depression, 00;31;37;11 - 00;31;41;17 that after about six months, they we will try to wean them off 00;31;41;17 - 00;31;42;20 of their antidepressant. 00;31;42;20 - 00;31;44;23 Now, I'm not trying to play psychiatrist. 00;31;44;23 - 00;31;49;07 Just provide some hope that this could be an underlying cause. 00;31;49;10 - 00;31;51;18 Look, you know, if a person's taking that stuff 00;31;51;18 - 00;31;54;18 and they've got you know, they've got issues and now, you know, 00;31;54;24 - 00;31;57;04 they come in to get the bioidentical hormone treatment. 00;31;57;04 - 00;32;00;20 And then three weeks later, they're like, man, I really feel pretty good. 00;32;00;24 - 00;32;02;14 You know, I, 00;32;02;14 - 00;32;05;28 I wake up in the morning at Sunshine and I'm feeling pretty good about things. 00;32;06;10 - 00;32;07;15 What's different? 00;32;07;15 - 00;32;11;09 One thing you did the bio identical hormone replacement 00;32;11;09 - 00;32;12;19 and you did testosterone. 00;32;12;19 - 00;32;14;04 And now you're bringing your body 00;32;14;04 - 00;32;18;05 back to a balance where it was when you were 35 or 40 years old. 00;32;18;23 - 00;32;19;22 That's right. 00;32;19;27 - 00;32;23;18 And I even have a nutraceutical line of products 00;32;23;18 - 00;32;26;17 that are all bioidentical supplements. 00;32;26;18 - 00;32;31;11 So let's let's get you off of the Trazodone sooner and use 00;32;31;12 - 00;32;35;18 and use a natural substitute while we're getting your hormones back up. 00;32;36;09 - 00;32;37;07 There's even and 00;32;37;07 - 00;32;41;00 there is even a new product on the market that is going to be used in acute 00;32;41;00 - 00;32;45;06 cardiac events that can actually start repairing blood vessels. 00;32;45;06 - 00;32;50;22 And that's something that's only provided by a certified Biote provider. 00;32;50;25 - 00;32;55;20 So we might actually be saving people's lives in the cath lab sooner than later. 00;32;56;02 - 00;32;56;27 That's remarkable. 00;32;56;27 - 00;33;00;12 I mean, one of the things I marveled at doing this show over the last sixteen 00;33;00;17 - 00;33;04;09 years, and I ask this question often when I have doctors on because it's such 00;33;04;09 - 00;33;07;13 you probably should get a doctor on here to spend the time with us and share 00;33;07;14 - 00;33;08;12 with the audience 00;33;08;12 - 00;33;11;11 what a difference in where we are today or where we were ten years ago. 00;33;11;20 - 00;33;13;29 So talk a little bit about that as we go to close the show. 00;33;13;29 - 00;33;17;04 I heard it because, I mean, this whole thing with bioidentical hormone 00;33;17;04 - 00;33;19;20 and where we are today versus where we were ten years ago. 00;33;20;04 - 00;33;21;14 What a difference. 00;33;21;14 - 00;33;24;26 So bioidentical hormone therapy is not new today. 00;33;25;06 - 00;33;29;08 Everyone thinks it's new because there is no awareness out there. 00;33;29;09 - 00;33;29;17 Right. 00;33;29;17 - 00;33;34;23 I think where we were ten years ago was that it was mainly synthetics 00;33;35;01 - 00;33;38;06 and all of the the big cancer scare 00;33;38;22 - 00;33;41;20 hit like a nationwide attack. 00;33;41;24 - 00;33;42;04 Right. 00;33;42;04 - 00;33;45;02 And we are still in the aftermath of that. 00;33;45;19 - 00;33;51;28 So today it's all about creating the awareness we didn't even touch on. 00;33;52;17 - 00;33;54;13 Thyroid replacement we didn't talk about. 00;33;54;13 - 00;33;55;13 We didn't touch. 00;33;55;13 - 00;33;56;11 Get you back in here. 00;33;56;11 - 00;33;58;26 We didn't talk about migraines. We didn't talk about thyroid. 00;33;58;26 - 00;34;00;05 We didn't talk about Botox. 00;34;00;05 - 00;34;03;03 I mean, there's a whole other list of things to talk about here, 00;34;03;10 - 00;34;07;14 but I think, you know, what we did today here again, the idea behind 00;34;07;14 - 00;34;10;23 this broadcast is to bring education and information to people 00;34;10;23 - 00;34;13;13 and to get them to say, hey, you know what? 00;34;13;26 - 00;34;14;25 That's me. 00;34;14;25 - 00;34;16;29 I'm the guy that has got the belly fat. 00;34;16;29 - 00;34;19;24 I'm the guy that's getting up four or five times a night to go pee. 00;34;20;12 - 00;34;20;27 Yeah. 00;34;20;27 - 00;34;23;28 You know, my moods have been kind of crappy lately. 00;34;25;21 - 00;34;26;22 Yeah, I really do feel like 00;34;26;22 - 00;34;29;15 I'm going to crash and burn after I eat lunch, you know, etc., etc.. 00;34;29;26 - 00;34;31;03 Oh, I'm talking to you. 00;34;31;03 - 00;34;33;25 Well, there's a solution to what we're saying. 00;34;33;29 - 00;34;34;14 That's right. 00;34;34;14 - 00;34;38;28 The treatment process is so easy for you as the patient. 00;34;39;04 - 00;34;42;20 The biggest challenge for me is just what my mission 00;34;42;20 - 00;34;47;14 is, to just create the awareness that we can all age 00;34;47;15 - 00;34;50;28 more gracefully and have a much better quality of life. 00;34;50;28 - 00;34;54;24 And that can start with people as early as in their thirties. Yes. 00;34;55;02 - 00;34;57;15 Not just all of us out there that are chronologically challenged. 00;34;57;15 - 00;34;58;21 We're talking to all of you. 00;34;58;21 - 00;35;04;15 And including you could be 90 years old and still be a candidate for this therapy. 00;35;04;24 - 00;35;05;24 That's remarkable. 00;35;05;24 - 00;35;07;16 You know, again, you know, I would I 00;35;07;16 - 00;35;09;25 and I really appreciate you coming on here today and doing this. 00;35;09;25 - 00;35;11;23 I think we got a ton of information out there. 00;35;11;23 - 00;35;13;16 Like I said, I'd love to get you back. 00;35;13;16 - 00;35;17;09 You know, when I do these kind of shows, people, the idea of it is to educate 00;35;17;09 - 00;35;20;17 you and give you information about something you don't know about. 00;35;20;17 - 00;35;22;07 Okay, you're not going to see this. 00;35;22;07 - 00;35;23;25 You know, in the paper, if you read a paper, 00;35;23;25 - 00;35;25;00 whatever the case may be, 00;35;25;00 - 00;35;26;12 you know, there's so much online 00;35;26;12 - 00;35;28;26 crap out there that you don't know what's real and what's not real. 00;35;29;18 - 00;35;32;08 The idea behind this is to give you that kind of information 00;35;32;08 - 00;35;34;17 so you can make good decisions and do things 00;35;34;17 - 00;35;38;00 that are going to improve your health and give you a better quality of life. 00;35;38;01 - 00;35;39;26 Doctor, thank you again for coming in today 00;35;39;26 - 00;35;43;21 and doing this doctors website is physicianaestheticspecialists.com 00;35;43;26 - 00;35;46;20 00;35;46;26 - 00;35;50;18 The phone number 913 706-0088. 00;35;50;25 - 00;35;52;28 And now I leave you with this thought from Dr. 00;35;52;28 - 00;35;54;04 Martin Luther King. 00;35;54;04 - 00;35;57;09 Americans must learn to live together as brothers and sisters, 00;35;57;21 - 00;36;00;17 or we will surely perish together as fools. 00;36;00;27 - 00;36;02;19 Sure, words were never spoken. 00;36;02;19 - 00;36;05;15 Thank you for listening to America's Healthcare Advocate broadcasting 00;36;05;15 - 00;36;10;07 here on the HIA Radio Network, coast to coast across the USA. 00;36;10;08 - 00;36;11;20 Goodbye, America.
